TWMB@Grady Family Info Sheet - English

Providing families with an information sheet during a prenatal visit affords families an opportunity to review and reflect on the information covered during their appointment. Having the information sheet available in several languages will further reinforce messaging for linguistically diverse families. This is an example of the Family Information Sheet used by TWMB@Grady. The Itegration Team partnered with Translatioin Services to translate this sheet into Spanish, Amharic, and French, the three most used languages used by Grady patients other than English.
