TWMB@Grady Patient Discharge Questionnaire

Utilize patient surveys to track integration progress and identify areas of focus. A family discharge questionnaire is an opportunity to collect additional programmatic evaluation data, monitor family adoption of TWMB practices and principles, record family feedback regarding their experience with TWMB coaching, and provides an opportunity to reiterate key messages prior to discharge. This is the Parent Discharge Questionnaire used by the TWMB@Grady Integration Team.
